My appendix was removed three years ago. Surgery on left knee and ankle at 22 after tearing ALC and MLC and shattering several bones in ankle due to baseball injury.

I take a men’s multivitamin and drink one cretin protein shake daily.

yes, I consume over 4 alcohol beverages daily, recreational use of marijuana for the past 15 years. I don’t smoke daily, but sometimes I will do it a couple times a week and then some months not at all. I’ve never failed a urine test, though once I did buy clean urine just to make sure.

Psychiatric Screening

I pretty much hate everything. I’m stressed out and angry, nothing seems to mean anything now that I have that stupid arrest on my record. Sometimes I get so mad I don’t remember anything till I wake up the next morning with blood on my hands. Part of my probation is getting inpatient treatment for my anger, I’m not sure what’s really wrong with me but something is. Normal people don’t react the way I do, they don’t explode if someone brings them the wrong coffee or if their baseball team loses. At least not the way I explode.

Have you been diagnosed with any psychiatric or psychological ailments? Please list any diagnoses below, and the treatments or medications prescribed to you. Please include name of medications, dosage, and number of doses per day:

As a kid I had several doctors tell my mom I had Intermittent explosive disorder and recently I’ve been talking to a psychiatrist in Cincinnati, it was kinda my boss’ idea so I could keep my job after my last freak out. Dr. Alexander said I’m suffering from post-traumatic stress from the injury as well as IED. Its not like I was a soldier or something, I don’t know how losing everything is equal to what those guys have. I don’t take the meds he gives me, but I do try to come to appointments at least every week.

Have you ever been hospitalised or referred to regular outpatient care due to these ailments or associated incidents? If so, please note where, and at roughly what age:

Two months ago I was held in the psych ward of University Hospital in WVU after smashing a man’s face in for making fun of the Bearcats. He was a dick but I didn’t mean to shatter his eye socket. Ummm I was in and out of psychologists as a kid but I hated what they had to say and mom and dad didn’t exactly hold me to it since I was so good at baseball. They said no school would want a head case. I was often getting in fights for no reason; I remember when I was 8 and some jerk stole my pencil on the school bus I ended up smashing his face into the window so hard it cracked it. The kid ended up needing over 40 stitches and I had to go to a different school. After that they gave me a few medicines, I’m not really sure what, and made me talk to some head shrinker but soon after the appointments started getting in the way of training and dad said I didn’t have to go anymore. Off and on after incidents they’d promise I’d get help or blame it on just being competitive. I dunno if anything’s really wrong with me but I know I do a lot of shit I don’t mean to. I just get so fucking pissed sometimes.

How have these ailments affected you and your life? Are there any major life instances you feel have been directly affected by these ailments, such as suicide attempts, criminal activities, etc?:

I’ve been arrested 4 times for assault and battery, the most recent two months ago. It’s made it hard to maintain a job or friends or anything. I’ve always had a temper but it got worse after the accident and the surgery. Anything that reminds me of that day can set me off, normally it’s not so much anger as its guilt. When I feel guilty I drink, when I drink I get angry. It doesn’t makes sense but it’s all I can seem to do anymore. I submerge myself in baseball even though ever second of being around it kills me inside. I can’t let go of it, I can’t walk away from the only thing I’ve ever known even if the sound of a ball of a bat makes me want to hide in the corner and cry. Sometimes in those moments it’s like I’m back on the field, rounding that base. The ground crunching beneath my feet, the wind against my face as my chest heaves…I just. I miss it. I hate it. I need it.

What is your social life like? Do you have many friends or relationships? How are your family relationships?:

I mean I have friends sometimes when I’m not losing my shit. And my family kind of tolerates me when they have to. Chicks dig me, but I don’t keep them around for more than a month or two before they piss me off. I’ve only hit a few of them, so I’ve tried to keep it to just sex.

Do you believe your life circumstances have contributed to any ailments? If so, what circumstances, and why do you feel they have contributed?:

Everything was great till I took that bad slide into second base. I was playing minor league ball. I had a huge contract with the Indians and was going to be moving up to the major soon. But I fucked it up and now I’m damaged goods. No one wants an outfielder with a bum knee. I lost everything. Money, love, friends, my life that day.

If you could change one past event that has happened to you, what would it be, and why?:

I would’ve slid head first. I wouldn’t have tried to leg it out, there are too many what ifs. What if I wasn’t such a fuck up. What if I was smart and had something other than baseball going for me? What if I would’ve paid attention to my parents and actually gone to college…I think I would just change everything and give back my wasted talent.

Do you wish to rehabilitate from your ailment(s)? If so, how do you feel this would best be accomplished?:

Well fuck yeah I want to get better. I don’t want to hate looking in the mirror, I don’t want to bash people in the face for looking at me wrong, I don’t want to be so damn broken. I just need to get away. Away from everyone who reminds me of what should’ve been.

Environmental History

Where did you grow up? Please list the location(s) and describe what it was like growing up there:

I had a great life. I grew up in Cincinnati, played baseball since I was old enough to hold a bat. I idolized Ken Griffey Jr. and always knew that I was better than all the other kids on the team. Travelled a lot with select teams and held win the state championship my junior and senior year. Always had a temper, was over competitive sometimes, but I could at least try to control it. I mean I never freaked out and hit someone with a bat or anything, but I did get into fights when people acted stupid. Been ejected out of my fair share of games as well…

What was your family life like? Did you spend much time with your parents? Do you have any siblings? If so, what are your relationships like?:

I have two younger sisters, they love me but they’re disappointed how I turned out, everyone is. My family spent a lot of time together growing up, following me from tournament to game to travelling team…They always had my back and I failed them. My talent was supposed to fix everything. Keep us financially secure, instead I ruined it.

What was school like? Did you have any problems? Did you enjoy school? What were your grades like?:

School was okay. I was popular, had friends, decent grades. I carried a B average while varistying in three sports. People knew I could get angry, but the fact I’m good looking got me out of trouble…or into it depending on who you ask.

Did you engage in any extracurricular activities, such as academic, artistic, or sporting clubs?:

Yeah, I played baseball, basketball, and football.

What was work like? Did you have any problems? Did you enjoy your work? What were your coworker relationships like?:

I was a minor league baseball player til I blew out my knee. After that I started doing work on the inside, stats, management, working my way up the ladder hoping someday to get a real job with the Reds. I got along fine with people as long as they did their jobs and didn’t slag stuff off on me. Sometimes I’d lose my cool and yell and I did make a few of the receptionists cry a couple times, but really it was their fault for forgetting how important that meeting was. One time I was put on a sabbatical for throwing a microphone out of the booth after one of the interns forgot to give me the correct stats for a game. I promised I’d get help then, but I just stopped drinking for a few weeks and said everything was fine.

Have you ever been convicted of a crime or misdemeanour? If yes, please explain, list conviction, and list any sentences associated with convictions:

I have been arrested four times, the first three the charges were dropped, this last time though is what got me sent here. It’s part of my probation for beating the shit out of some guy. I swear I didn’t even realize how bad it was till I saw him in court.

Lastly, please tell us about yourself. How do you feel about yourself and what you have done with your life? If you have committed crimes, how do you feel about those now? What are your hopes for the future?:

I know I fucked up. I know I have a temper, I just want to stop doing these things before I kill someone. It’s not that I’m a bad guy, I just can’t help it.

After reviewing Nolan’s admission forms I feel like there is a bit more to add. Nolan has had not let on to just how bad things are in his life to those that he does not trust. His application in my feeling is a direction example of this. Often he holds things inside from embarrassment of his actions, especially when it pertains to his violence. As a child Nolan was suspended from school several times for fighting, once he attacked a teacher with a yardstick, breaking it across his face. This was in the 5th grade, and things have not truly improved at all since then. There are only a handful of times I have gotten him to talk about his “explosions” and I feel they might be helpful in his continuing care.

I have realized that Nolan does not like to talk about his relationships with women. The two times he has gone into depth with them the stories were very similar and I feel that it has probably been the case with the majority of his relationships. Soon after graduating high school he became involved with a young lady (I will refer to her as X as to keep her privacy). The relationship seemed in his words “perfect” but several months into the relationship he found some texts on her phone which “set him off.” He claims he remembers little of the actual assault but yet could give specific details when pressed. He claims she admitted she was sleeping with someone else and he destroyed her living room. He went as far as to throw a chair through her television before punching X. The neighbors heard the disturbance and the police were called, however X dropped the charges. A similar occurrence happened when he was twenty four years old. Since then he claims he doesn’t date and is only sexually involved, preferring one night stands.

The most notable though was his sophomore year of high school ball. After a night of drinking he showed up to the game still hung over and in a foul mood. Taking it out with yelling and screaming at anyone who dared looked his way the coach had enough and benched him for the game. Nolan ended up destroying the locker room and very nearly punching his coach in the face. He suffered a 10 game suspension for the disruption and held a grudge with the coach for the rest of his time there, going as far as to pick fights and rallying the team to harass the man till he left his job.

Baseball was an outlet at first but soon he felt that it was more of a requirement for his father to love him and for his family to have what he felt they deserved. Throughout his childhood his parents placed priority on him and his talents, pushing him far beyond what most parents would deem acceptable. Placing extreme stress on him they noticed he’d have fits of rage, mostly directed at umps or coaches he felt didn’t do what was best for him. Calling a strike that just barely touched the outside edge could send him off his handle, throwing a bat and swearing loudly, often times getting him ejected from the game. The outbursts were horrible and violent; at once point a teammate teased him for missing a sliding catch that bounced out of his way letting a run score. Nolan retaliated by taking the kids bat and breaking it across the water jug screaming that if he said another word it would be his face he put the bat across next. Coaches pass it off as “competitive” or “enthusiastic”, however the results have him suspended for a few games each time. Nolan believes his sole purpose in life was to be a major league baseball player and since the accident, which I can hardly get him to talk about, he feels he has no purpose in life. As almost a punishment he has taken a job on the sidelines of the sport which although he is talented at I do not feel is conducive to his recovery.

Mr. Cooper has had many run ins with the law due to his IED, however a substantial amount of money and a lot of leniency with the police department due to his status as an athlete has given him a sense of entitlement and this probation requirement has him very on edge. In my professional opinion I do feel that inpatient treatment is far better suited to Nolan due to him being a threat to himself and others. Hopefully your facility can have more progress with him than I have.

Nolan isn’t a bad guy in his eyes; he’s always just had a temper. To those around him he’s known to explode at any moment, especially after his accident playing baseball. Taking a nasty slide into second trying to take out the short stop Nolan’s injuries were bad; his ankle snapped in several places he completely dislocated his knee and tore both major ligaments; he knew he’d never play at the major league level again. Since then he’s had a lot of guilt over everything he lost, feeling like he completely let his family and friends down he turned to alcohol which only exacerbated his violent temper. He’s been able to keep work as long as he doesn’t have to deal with too many people face to face (ironically he wants to be the GM of the Cincinnati Reds someday) but one weekend at a football game for UC and WVU Nolan got into a fight with another fan. Nolan would have beaten the guy to death had it not been for quick acting by the stadium’s security. He did end up however shattering the man’s eye socket and breaking his arm. As the police were on scene it became clear that Nolan wasn’t in his right mind, instead of throwing him in jail they kept him for 48 hours psych hold where he spoke to Dr. Blankenship. It seemed to be a weight off Nolan’s shoulders to let it out but it also opened his eyes to just how volatile his behavior can be. Opening up about his past Dr. Blankenship was able to give a firm diagnosis of IED. His parents helped secure one of the best lawyers in the state and he was give 5 years’ probation with the deal of inpatient treatment for his intermittent explosive disorder and Alcohol Abuse. Instead of being transferred back to Cincinnati they are holding him in West Virginia, due to that being the site of the crime.

  • Thank you for apping! Nolan is just about ready for the haunted halls of Hallowsgate, but there are a few quick bullet points to cover first. So stick with me and we'll get right to it!
  • Nolan is said to have abused marijuana for fifteen years while participating in minor league baseball, however athletic programs typically have a very strict drug policy and will kick players out for abusing. How was Nolan able to hide his habit that long? I recommend either cutting out the drug abuse, or keeping to after he was injured.
  • We need to know more about Nolan's disorders and how they have affected his life. We get some detail from his childhood, but not too much more. How did Nolan's IED impact his baseball career? How did he handle the stress of winning and losing? Given the volatile nature of the disorder, there's almost no way Nolan wouldn't have had incidents with teammates, coaches, fans, etc. We also need to know more about his PTSD, like what his triggers are and what his episodes are like. There is also one mention of a suicide attempt, but no information is given. If Nolan did try to kill himself we need to know when, how, why, and why it failed. Also, why didn't he try again?
  • As a final note, I recommend having Nolan seeing a psychiatrist at the time of his arrest, if only to help get him to Hallowsgate. Unfortunately for him, most drunk, violent men end up in prison instead of getting the help they need. If he has a current diagnosis and is receiving treatment or on medication, I feel it would be more likely that he'd be sent somewhere to actually get better rather than being sent to the slammer.

  • First off, we would like to thank you for expanding more on Nolan, however we just have a few more points to touch on. Have no fear! This won’t take long at all! Very Happy
  • The first point we’d like to address is that we still feel that Nolan’s disorders have room for improvement. There is a time between the ages of 18 and 25 that are unaccounted for with his IED, and we would like to see that expanded upon. As a suggestion, we recommend focusing more on the IED and less on the PTSD; or even dropping the PTSD all together, as it is the IED that is the main disorder. However, if you would prefer to keep the PTSD, we need to hear about the symptoms and what is being done about them.
  • This leads into the next point! The information on his IED and PTSD, if you choose to keep it, would need to be in character, rather that out of character, otherwise Hallowsgate’s therapist do not know what is going on.
  • Thank you for sticking through this far! It’s not too heavy of a pend, just a few minor adjustments, and Nolan will be ready to use his grit and determination to help keep Hallowsgate safe! Smile

Therapy Notes: I began the session welcoming each participant and allowing them to choose a seat in a circle within the recreation room. Tally was disrespectful from the start, referring to me by my first name “Rosie” although was altogether pleasant otherwise. Adriana appeared disinterested in the session, as well as Nolan. Lauren was quiet and unassuming upon entering. Jamie appeared irritated and disinterested. We began the session introducing ourselves, and it seemed most were tired of this exercise (as it appears in most sessions—there should be a note regarding who has worked with whom). Shortly thereafter, we began the visualization exercise. After explaining to several members of the group what visualization was—with some help from Tally, albeit critical and rude—we began. I asked them to imagine themselves: You are lost at sea on a stormy night. You see a glimmer of light leading you to land. If you row hard, you can make it. Someone waits for you with a warm meal, dry clothes, and a place to rest. I then stated that we would be sharing what we had seen, and proceeded to go around the circle. Adriana shared that she saw Andrew, and that he reminded her of “Patience, stability....old spice aftershave and that sandalwood orange soap he always used..." Unfortunately, Nolan appeared to have a flashback to the time of his injury. While we woke him, Tally continued to agitate other members of the group. He left of his own accord, cursing the session and storming off. Dr. Finn McAlister was present to help with his dismissal. Jamie Gibbs also decided to leave, declaring the session worthless, and stating that I did not care about her. Tally revealed that she saw herself on the shore, and that she didn’t think that she would make it. Lauren went on to reveal she saw her husband and children, that "Kenneth was holding them in his lap. Kenny and Natalie were holding their arms out for me. They need their mother." Tally and Lauren had a positive exchange, and after this I dismissed the group

Additional: Nolan was completely inconsolable after his outburst. His flashback marks that his thoughts of the past triggered being back playing baseball.

Follow-Up: Reevaluate Nolan’s medication, and begin working immediately on his anger management. Explore why, when thinking about his source of strength, he immediately drifted back to his incident.

Incident Notes: Mr. Cooper and Ms. Tansy were involved in a physical fight in the Ballroom. Upon questioning, it was clear that both patients were at fault for this altercation. Ms. Tansy and Mr. Cooper was simply dancing with each other, and there weren’t any problems until Ms. Tansy kissed Mr. Cooper, causing the man to draw away. It was reported that something caused Mr. Cooper to scream, to which Ms. Tansy hit him out of annoyance. Mr. Cooper then pushed Ms. Tansy, and she goaded him into hitting her. Mr. Cooper responded by back-handing her on the cheek and kicking her in the stomach, and Ms. Tansy curled up on the floor to defend herself. I arrived when Mr. Cooper was kicking Ms. Tansy, and I subdued him, sending him to isolation shortly after. I radioed in for Ms. Tansy to receive medical treatment for her injuries.

Additional: These two should be kept away from each other until the issue is resolved with the help of the psychiatric team. What made Mr. Cooper scream and not Ms. Tansy was very unclear upon questioning. Mr. Cooper lashed out several times while struggling against my hold on him, using various profanities and calling me a ‘wetback’.

Follow-Up: In future therapy sessions, I would suggest coaching Mr. Cooper on his view on relationships with women and recognizing when and why his anger tends to flare up. Additionally, it would be wise to help Mr. Cooper understand the gravity of his words- the racial slur did not have any effect on me, but it most certainly will have an effect on others. Helping Mr. Cooper understand that Hallowsgate is a place for patients to heal and not to breed a culture of hate will be imperative for the well-being of himself and the entire patient body.
