Please explain why you are applying for the position(s):I am applying for the position because I would like to experience a new environment while performing the job I love very dearly.
Why do you feel you would be the best candidate for the position(s)?:I feel like I am the best candidate for the position because I have a very enthusiastic perspective and can push patients when needed. I can also pose as an example for patients who feel as if therapy is a “waste of time” as I have been through both physical and psychological therapy, and have become a better person.

What other qualifications or skills do you possess which you feel will aid your performance in the chosen position(s)?:As part of my Doctorate, I obtained and have kept a CPR certification. I am also skilled with computers and basic word and powerpoint. I also know American Sign Language.

If so, where and when?:I have not worked in a psychiatric facility before, however I have interned and worked at Three Rivers Physical Therapy for the past seven years. I interned from 2003 until 2005 in which I came on full time. I also worked sports medicine for Shenandoah University Women and Men’s soccer teams after my graduation.
Please describe your most recent employment experiences, including your positions and responsibilities there:As a physical therapist at Three Rivers, my responsibilities included assisting patients in their quest to regain or improve mobility after surgery, injury, or accidents. I also assisted players on the field during in-game injuries, along with the doctors on call.
How do you think your previous education and employment experiences will aid your performance within the chosen position(s)?:In my employment and education, I have come across many different types of people from every walk of life. This gives me perspective in that not every patient is textbook, and not every patient is going to behave the same. I have also learned to work in the heat of the moment due to the sports medicine experience.

What methods would you employ in order to relax during times of stress, and how effective do you think these have been in the past?:I exercise regularly, which helps me to relax and blow off steam. I enjoy a pick up game of soccer, or jogging.
Please list three traits about yourself that you consider negative, and three traits about yourself that you consider positive:I tend to run a little hot headed at times, however, usually not at patients. However, I am compassionate and have a rather large heart (Although it’s well hidden, and by this, I tend to look after those in small ways, however, I try my best not to be overbearing. I tend to be a little more outspoken than I should, but I am open minded. I am very independent, which can turn into a negative thing when I refuse to ask for help on anything. But then again, this can be a positive attribute, as I am not one that is needed to be chased after and looked out for.
Describe one event in your life that you would change if you could, and why:I prefer not to dwell on the past, and make the best out of what is to come.
If a superior gave you a task to complete, to which you were wholly, morally opposed, what would you do?:I would question the necessity of this task; or tell my superior that is against my moral code of conduct and I cannot complete the task.

Environmental Screening
Where did you grow up, and what was it like there?:I grew up in Richmond, VA until I was seven, in which I moved to Yorktown, VA. Both places were nice, I lived in a decent neighborhood in Richmond, and in Yorktown, being on the bay was a nice place to live.
What are/were your social interactions like? Do/did you have many friends and/or relationships?:As a child, I had friends from school and from my neighborhood. However, I did not have terribly many friends, choosing to keep those I was close to, instead of having a lot of acquaintances. I was also the target of childhood teasings, however, this was quickly extinguished and taken care of.
I’ve dated off and on since High school and college, but I’ve never been married. No children.
What prompted you to pursue the career path you are now following?:I broke my arm during my Junior year of college while playing in a soccer game and the break required surgery and physical therapy, and I found I enjoyed the aspect of helping people.

Out of Character Section
What are they not telling us? What secrets do they have to hide? What back story are we not hearing? This is where you can tell us all the things your character wouldn't put on an application, or others don't know.:While truthful, Abigail is very vague about her past for many reasons. Her parents passed away in a car crash when she was seven, and due to its location and the circumstances of the wreck, Abigail was not found for at least an hour after the wreck. Abigail escaped with cuts from flying glass and debris, and she was rendered partially deaf due to damage to her auditory nerves from the collision and whiplash. She is 70% deaf in both ears. Her parents died upon impact due to an airbag malfunction, and the speed of both cars involved. This is the source of her moving to Yorktown where Free Will Baptist Home for Children took her in as Abigail had no family in the area. Both grandparents were deceased, and Abigail’s aunt refused to take on the precocious, rambuctious and special needs child.
She was interviewed four times for adoption and denied four times, which leads her to believe that it is a sick world where parents can pick and choose their kids, as if they were in a clothing shop trying on new shirts.
For a very long time, Abi did not do well in school and it wasn’t until her social worker and the sisters over the church-run foster him requested the state for her to see a child psychiatrist did she get better. It still leaves her emotions a little raw, but this also spurred her independence, and fueled her feisty attitude. She learned quickly in the children’s home, and as a child from a children’s home in school, that she needed to stand up and speak her mind and defend herself because there was no one around to do that for her. It wasn’t until high school that her grades leveled out enough to send herself to college.
She learned American Sign Language through the children’s home, and by a state appointed case worker. She also learned to lipread, and has since been outfitted with ear pieces that increase her hearing however, she still has a hard time hearing, and often insists that people face her when talking.
