Employment Interview
Please list position(s) for which you are applying:Front Desk Receptionist
Please explain why you are applying for the position(s):The thing that really attracted me to the position and encouraged me to apply for something out of state is the included housing. I’ve been trying to better myself and get a stronger education, but with rent costs to live in a safe area I’m struggling to save up for school. Not having to worry about that aspect would enable me to save up and take online courses more frequently.
Why do you feel you would be the best candidate for the position(s)?:Reception work is something I am very comfortable with and have experience doing. I’m very organized and have a great phone voice. I am approachable, take direction well, and comfortable in a mental health environment.
Do you posses the necessary qualifications for the position(s) for which you are applying? If yes, please list them below:I have my administrative assistant and first aid certifications both from the University of Arkansas Community College at Hope.
What other qualifications or skills do you possess which you feel will aid your performance in the chosen position(s)?:As mentioned above I do have my first aid certification. I also have personal experience with mental health facilities that will help me to be comfortable in yours.
Have you worked in this type of position or facility before? If so, where and when?:As a receptionist and office manager at the Imperial Dinner theatre for the past five years.
Please describe your most recent employment experiences, including your positions and responsibilities there:Imperial Dinner Theatre – Character Wait Staff and Ticket Sales, then receptionist, then office manager. I kind of got my foot through the door with my performance experience. Then transitioned into the office and worked my way up the ranks there as much as I could with my education level. So at first I was costumed and playing and character that suited the mood of the show while serving. Then I was handling money and being the public face. Then I was doing the PR and helping to manage the office and finally I was managing the office solo.
Before Imperial I worked at a restaurant as a hostess. Seating tables, busing dishes, pouring water, things like that.
How do you think your previous education and employment experiences will aid your performance within the chosen position(s)?:My administrative assistant certification makes me perfectly prepared for a front office receptionist position and my previous employment will only add to that, I am excited to start a new challenge in my career by helping in the smooth operation of a facility more important than a dinner theatre.

Psychological Screening
Please describe yourself to the best of your ability, in only five words. ('And' will not count towards these.):Determined, poised, hard-working, organized and healthy.
What methods would you employ in order to relax during times of stress, and how effective do you think these have been in the past?:I have been a dancer all my life. And while it’s no longer healthy for me to purse it as career (I have shin splits so I need to be wary and use moderation) it’s still something I enjoy. I dance twice a week in the modern style and during times of stress I often end the day by running though a bar routine. I find them very meditative and it encourages good posture to counter the effects of sitting at a desk for much of the day.
Please list three traits about yourself that you consider negative, and three traits about yourself that you consider positive:Negative: Critical, Gullible, No Sense of Direction.
Positive: Driven, Helpful, Kind
Describe one event in your life that you would change if you could, and why:Like many aspiring professional dancers: puberty and the changes in my body type threw me for a serious loop. At fifteen I checked into an inpatient treatment facility for Bulimia Nervosa. I am glad I got the help I needed to get healthy, but obviously the path I was on before if not one I remember fondly. If I could change anything, I never would have allowed myself down that path to begin with. It’s hard to look back and see a teenage version of myself punishing herself for have the curves I pride myself on now.
If a superior gave you a task to complete, to which you were wholly, morally opposed, what would you do?:Ask for further explanation. If any hesitation I had could be cleared up with better communication that would be ideal.
If you caught a friend and co-worker stealing from your place of employment, breaking rules, and/or endangering the safety of others, what would you do?:I would confront them, see if there was some bigger issue. If they failed to explain themselves I would have to report them to our superior.
If your application for employment to Hallowsgate Hospital is approved, will you agree to sign a full non-disclosure agreement, knowing that you would be unable to talk about any treatments or the patients with anyone not also employed by Hallowsgate Hospital?:Yes.

Environmental Screening
Where did you grow up, and what was it like there?:I grew up in West Hollywood, California. Not the best home for someone with body issues.
What are/were your social interactions like? Do/did you have many friends and/or relationships?:I was an extremely dedicated dancer in my youth so most of my social interaction was either with my family, instructors, or fellow dancers. In treatment and my adult life I have met and befriended a wide variety of people that have really expanded my world view.
What prompted you to pursue the career path you are now following?:Being a public face for a business or facility is one that allows me to use the skills I had from performing outside of theatres, auditions and competitions. I’ve had an interest in pursuing medical administration and/or nursing for some time, so this position would be a good next step in that direction.
Were you ever a member of the armed forces or of a law enforcement or security agency? If yes, please list rank and branch, or company name and location:No.
Do/Did you ever abuse controlled substances, including but not limited to alcohol, prescription medications, street drugs and tobacco?:No.
Have you ever been convicted of a crime? If yes, please list conviction(s) and served sentence(s) below:No.

Out of Character Section
What are they not telling us? What secrets do they have to hide? What back story are we not hearing? This is where you can tell us all the things your character wouldn't put on an application, or others don't know.:So, there are definitely so details of her life that Maggie is dancing around. For starters she wasn’t
just an avid dancer. She was a champion youth Latin ballroom dancer, a repeat pageant victor and had a few minor TV/movie roles. All thanks to her over-whelming stage parents playing her like a marionette.
Bulimia, though something she struggled with, was not her initial reason for entering treatment, but rather a sealed juvenile shoplifting record. In treatment she was diagnosed with HPD with an emphasis on standards for her appearance: thus resulting in the Bulimia and shoplifting. Her doctor Rachael Vincent managed to make great strides with Maggie, especially after encouraging her other interests and limiting Maggie’s contact with her family.
Now largely recovered, Maggie is just trying to move on with her life, but a few bits of her issues remain. She has difficulty eating full meals without “nausea” preferring to eat healthy snacks through-out the day, usually with a container of nuts or carrots at her desk. She remains critical of her appearance and while she'd happy with her weight and looks discussions of things like diets or attire with friends or co-workers will later be followed by time spent scrutinizing herself in the mirror. But her most prominent problem at this point is her loneliness. Maggie is easily seduced by both the promises of love, friendship, and approval. She frequently finds herself in relationships that inflict varying levels and types of abuse upon her. Heartbreak and general angst is a very good friend of hers.
