Medical History

Do you have any ongoing medical issues for which you require treatment or medication?:

No.

Adriana has been diagnosed with Rapid-Cycling Type II Bipolar. She rejects her diagnosis completely. Insisting she is simply moody or perhaps has Premenstrual Dysmorphic Disorder. Adriana has been tested for PMDD and she has no symptomatology of such. Adriana wholeheartedly refuses to believe this and insists that she is not bipolar but ‘simply moody’.

Are you aware of any allergies? If so, please list allergy, age of onset and any medications or treatments you require or recieve:

Mold, age 2 and I just need to keep my nose out of it.

Have you had any surgeries or invasive procedures in the past? If yes, please list reason and approximate age of procedure:

I had my appendix out at age 11 and my wisdoms out at 23.

Do you take any medications or supplements daily? Do you follow any treatment plans? Please list medications or treatments, and reasons below:

Not by choice, I’m on a regimen of Depakote 125mgs, twice a day courtesy of Dr. Bianchi at CRC for something I don’t even have.
I wanted to take a moment to stress that Ms. Clarkson is not delusional, just headstrong and determined that she does not have any form of mental illness.

Do you use tobacco, consume alcohol, or use any other drugs including street drugs and/or prescription medications not prescribed to you? If yes, please list number of packs a day, number of drinks a day, and/or drugs consumed below:

I don’t drink very much. Although I am sure Dr. know-it-all it going to say I’m an alcoholic. I do enjoy weed, but it’s just weed it’s not like I’m snorting cocaine.

Ms. Clarkson has a history of alcohol and Marijuana abuse and 4 charges of drunk and disorderly conduct, the last of which--along with additional charges and a history of this behaviour-- being the reason she was court-ordered to this facility.

Psychiatric Screening

Please describe, to the best of your ability, your emotional and mental state of wellbeing:

I’m really moody, it’s disruptive but I handle it. I’ve always handled it. It’s not worth all the drama everyone makes it out to be. I’m perfectly fine, I suppose just feel things intensely, it’s my cycle but no one listens. And how do you Think I feel, I’m in one nuthouse and the people there are trying to ship me to another; I feel shitty.

Adriana’s emotional state is quite frankly, conflicted. She’s not naive enough to lack an understanding for why she is here but Adriana refuses to admit that she is bipolar and that it is having a direct effect on her life.

Have you been diagnosed with any psychiatric or psychological ailments? Please list any diagnoses below, and the treatments or medications prescribed to you. Please include name of medications, dosage, and number of doses per day:

Not answering.

Adriana has Rapid-cycling type II Bipolar Disorder. She was only given a formal diagnosis six and a half years ago, but I believe she has been living with bipolar her entire life. From what she has told me in sessions and what I have uncovered through research, Adriana has periods of hypomania where she is hyper focused, excitable to the point of irritability, financially reckless and hypersexual. They are followed by periods of depression where Adriana isolates, becomes highly pessimistic, her energy levels and interests drop along with quite recently the addition of suicidal ideations. These are also the times when her drinking becomes more uncontrolled and she is reckless.

When she is between severe mood swings, Adriana is an intelligent and insightful young woman if not a bit emotionally reserved and evasive. Her previous doctor (there was a seven months span before being court ordered to this facility where Adriana elected at the insistence for her girlfriend to see a psychiatrist.) prescribed lithium but the myriad of side effects made Adriana discontinue treatment. She is currently on 250mgs of Depakote distributed twice a day at breakfast and before bed.


Have you ever been hospitalised or referred to regular outpatient care due to these ailments or associated incidents? If so, please note where, and at roughly what age:

I’m here aren’t I? I was referred to Charleston Rehabilitation Center on a drunk and disorderly charge.

How have these ailments affected you and your life? Are there any major life instances you feel have been directly affected by these ailments, such as suicide attempts, criminal activities, etc?:

I've been diagnosed with bipolar something but if I really were bipolar and not PMDD or something, why would it take till I’m twenty-seven to get a diagnosis? I don’t buy it. I used to take my meds more often when my ex and I were a thing but she left and I don’t have to anymore. He prescribed fucking lithium like I’m some paint-licking sociopath. That stuff is poison and after getting almost every side effect on the label and no real response from Dr. Dickhead I stopped taking it.

Late diagnosis for bipolar disorder is normal for women of color, and considering Adriana’s impoverished upbringing it is no surprise it took so long for her to be diagnosed. Her previous psychologist, Dr. Amelio Giancarlo, simply wanted her to continue the Lithium for a full six months as they had originally negotiated before discontinuing the medication, Adriana stopped attending sessions and quit her medication after only four months. His notes show that while the side effects were severe, her moods were stable while she was under his care.

What is your social life like? Do you have many friends or relationships? How are your family relationships?:

My family’s aright, relationships could use work.

Adriana had a long-term relationship come to an end before she was court-ordered to our facility. It was defiantly a tipping point for her. She refused to talk about it further than what she mentioned above. With her previous therapist she spoke a little more openly about her family, primarily her mother who was very emotionally unavailable to her children and modeled many of the maladaptive coping mechanisms that Adriana relies on.

Do you believe your life circumstances have contributed to any ailments? If so, what circumstances, and why do you feel they have contributed?:

No.
Adriana’s life circumstance, from the impoverished early childhood, and adolescence filled with friends who were less than stellar models of behaviour and the late diagnosis of bipolar and her own refusal of everything from treatment to admitting she even has an issue are the reasons she is here today.

If you could change one past event that has happened to you, what would it be, and why?:

I would have told my mother I was a lesbian lot sooner if I knew that was what it would take to make that hypocritical alcoholic bitch stop talking to me.

Do you wish to rehabilitate from your ailment(s)? If so, how do you feel this would best be accomplished?:

This question is bullshit because there is absolutely nothing wrong with me; I have moods but what human being doesn’t?
As stated before, Adriana is only resistant, not delusional. She would like to have her life back; she is young woman in a facility with a curfew and nightly bed-checks. Ms. Clarkson ruffles under this level of supervision, she can see that compared to some of our more severe patients that she is higher functioning and believes the by comparison she should not be here.

Environmental History

Where did you grow up? Please list the location(s) and describe what it was like growing up there:

I grew up in Harrisville, West Virginia, I don’t think the population has cracked 1,300 yet. It was small, claustrophobic little town. It was the kind of place where you turn 18 and get the hell out of dodge before you get knocked up by some dude and start pumping out babies like a human geyser with the rest of the girls you knew in high school. Then again I knew pretty early that I wasn’t like the other girls at my school, they liked little boys and I never did that way.

What was your family life like? Did you spend much time with your parents? Do you have any siblings? If so, what are your relationships like?:

I had my Stepdad, Andrew who married Her. And my siblings who are all younger: Shaun, Jaida, Patrice and James. The series of sperm donating tricks She used to conceive us walked out ages ago. The last two are the only ones Andrew’s biologically related to but you wouldn’t know it because he treats us all like his blood.

Andrew took a shine to me, not in a sick way you assholes, he just thought I was smarter and better than most people believed. He called me on my shit when I tried to sneak out and be an overly rebellious teenager, not that I didn’t get away with things. But...he gave a shit, a fat lot more than I can say for My own ‘mother’.

As for friends I had a couple in grade school who left me in middle school, when I hit high-school I made new friends, yes they were the washouts but I needed friends who wouldn't judge me...and had nice weed. I was only here for the liquor, weed and friendship though; i never touched the harder drugs.

Adriana’s mother was not a prostitute, but she did have a series of boyfriends for most of her older children’s formative years, marrying when Adriana was thirteen. She has a demonstratively stronger relationship with her step father then her mother, he has written her a few times when she was here and the letters from her stepfather always lift her up. She is telling the truth about her addictions as well.

What was school like? Did you have any problems? Did you enjoy school? What were your grades like?:

I didn’t enjoy school much, I always felt kind of awkward there like if life was a dance I had two left feet; sometimes I was the center of everything, others I barely wanted to leave my room. I’ve always been moody, y’know? Just always. My grades sucked until junior high when the few friends I had abandoned me for other groups and just not to be around me.

With nothing else to do I would say I was doing my homework to avoid her boyfriends and get some peace so I didn’t have to play mom to my younger siblings for a little bit. Once Our mother married Andrew I didn’t have to play mommy to my siblings anymore, I got better at it the whole school thing. Turned out I was pretty damn smart when I wasn’t in a shitty mood and actually did the work. I started getting A’s in 8th grade for the first time. I had a thing for chemistry, and that took me pretty far.

Did you engage in any extracurricular activities, such as academic, artistic, or sporting clubs?:

Not at first. We were a really poor family, we didn’t have a money for extras, didn’t often have much for the essentials till Mother married Andrew, suddenly there was money for things like school supplies and jeans... we didn’t have to go to the church for those poor kid packs of clothes or school supplies. Andrew encouraged me to try out for things. I was in the 4H club and the science club before I left them junior year.

What was work like? Did you have any problems? Did you enjoy your work? What were your coworker relationships like?:

I started working at thirteen; when I wasn’t too moody would babysit, when I got older I worked in a store in town. Well a couple stores. Not like there were many to choose from the bosses were always jackasses and I would end up getting fired after only a few months. Once I got to college things were a little better, the scholarship gave me enough money that I didn’t have to work too often.
When I entered my first real job in an advertising firm things were alright, but then everyone started treating me like I was crazy. One bad day and all of a sudden, I’m getting write-ups and trips to HR and my bosses offices. If they would have just let me to the work instead of wasting my time with intra-office politics.

Adriana never seems to believe herself at fault for anything, minor or major. HR records show the she often missed work and performed poorly, and had issues with coworkers.

Have you ever been convicted of a crime or misdemeanour? If yes, please explain, list conviction, and list any sentences associated with convictions:

A lot of bullshit, nothing that I should have ever been charged for.

Adriana has had multiple charges against her. Mostly misdemeanors but they represent a pattern of disregard for the law and her own safety;

Ages 15 & 16, 4 counts underage drinking: fines totally $900 and 120 hours community service, Driver’s license suspended until age 18.

Age 17: Possession of Marijuana, less than 15g: $1,000 fine, 6 months’ probation to her parent’s care. She was also ordered to attend a minimum 3 months of Alcoholics/Narcotics Anonymous meetings.

Age 21, Drunk and Disorderly: No real damage to property, received community service, Stepfather promised she would not have further issues while at school.

Ages 24 & 26: Drunk and Disorderly: Fines totaling $600 and 60 hours community service, and stern warning from the judge about more serious charges if she continued this path. Ordered to attend one year of AA.

Age 29: Drunk and Disorderly, assault of a police officer, simple assault, and Resisting arrest possession of Marijuana: She slapped a woman, shoved a police officer and refused to get into the vehicle. When she was arrested they found marijuana in her purse and in addition to the fines totaling $4,000, Adriana was ordered to the Charleston Rehabilitation Center for a minimum of 18 months.


Lastly, please tell us about yourself. How do you feel about yourself and what you have done with your life? If you have committed crimes, how do you feel about those now? What are your hopes for the future?:

Being drunk in public a few times shouldn’t constitute a trip to a mental hospital. I want to get over with this crap and get another job and the hell out of West Virginia. Maybe head to California.

When she isn’t in a hypomanic or depressive state Adriana is an intelligent, self-possessed young woman. Her maladaptive coping skills and refusal to accept responsibility for her actions the consequences of them are the primary reason she is here. If she can accept she is bipolar, take responsibility for her life and the outcomes she is living with and conquer her addictions she can and will have a very bright future.

Adriana is being referred to Hallows gate because five weeks ago she was in a depressive state and attempted suicide by tonguing her Depakote and taking all of the pills at once. As she is coming to the end of her sentence here I cannot in good conscience say that Ms. Clarkson is rehabilitated enough to leave treatment, but she is beyond the current capabilities of CRC.

Out of Character Section

What are they not telling us? What secrets do they have to hide? What back story are we not hearing? This is where you can tell us all the things your character wouldn't put on an application, or others don't know.:

Yeah... there are quite a few potholes in Adriana’s story, mostly lies of omission but the real skinny follows.

For one, her suicide attempt was not because she was depressed. Her doctor is mistaken; Adriana tried to commit suicide because another depressive state was coming. She may not believe she is Bipolar but Adriana knows when one of her depressive episodes is coming and she hates them, she was trying to avoid it permanently.

For background Adriana is the oldest of five children, only she and her brother Shaun might have the same father. Their mother, who was a heavy drinker and had bad taste in men and issues with fidelity to her paramours’ for a very long time. There was no sexual abuse suffered by her or her siblings, but a lot of cruel words when Mom picked a loser as her latest love. Which happened roughly half the time. Adriana’s bipolar symptoms started showing up around preschool, but her mother who was only 19 at the time simply ignored it or punished her young daughter for being “bratty” Her mother is also where Adriana learned that when life doesn't work out how you want blame someone else, be it your ex, your parents, or your young children.

Adriana and her mother have not spoken in over ten years when Adriana disclosed the she is a lesbian. Her mother naturally took this as either a joke or a mark of poor parenting, classic self-centered behaviour for her mother, Adriana was infuriated that her mother would use her coming about as a moment for self-serving pity and severed ties with the woman, she is still in contact with her stepfather and siblings when she ‘remembers’ to contact them.

Adriana discovered Marijuana at thirteen, and has been unknowingly self-medicating with it since then for her ‘moodiness’ which we’ve all figured out by now is her previously undiagnosed bipolar disorder. She is a regular smoker

What she has not talked about so far is her ex-girlfriend, Amelia. The two met in college through mutual friends and after they graduated the two moved to Charleston where Amelia is from, it wasn’t like there was much Adriana wanted to return to in Harrisville. They were a couple for eight years, right up until a few months before she quit her job, but it was anything but a picture perfect relationship. Adriana’s drinking, up and down emotional state, refusal to take responsibility for her behaviours or regularly take her medication when she was finally diagnosed with bipolar disorder, along with being caught almost cheating was the death knell for their relationship.

Because she never takes responsibility for her actions, Adriana alternately blamed Amelia for their break up, and drank more. This resulted in the break-in’s, the post-relationship fights and finally her court-ordered stay at CRC. The diagnosis was Amelia’s idea, she convinced her, during a window when Adriana’s moods were even and Adriana was reasonable enough to listen to get some help, she was finally diagnosed, but the prescription of lithium was full of side effects, the one that made her finally refuse taking it was when she gained roughly forty pounds from the drug.

    Session: Group Therapy


Therapy Notes: I began the session welcoming each participant and allowing them to choose a seat in a circle within the recreation room. Tally was disrespectful from the start, referring to me by my first name “Rosie” although was altogether pleasant otherwise. Adriana appeared disinterested in the session, as well as Nolan. Lauren was quiet and unassuming upon entering. Jamie appeared irritated and disinterested. We began the session introducing ourselves, and it seemed most were tired of this exercise (as it appears in most sessions—there should be a note regarding who has worked with whom). Shortly thereafter, we began the visualization exercise. After explaining to several members of the group what visualization was—with some help from Tally, albeit critical and rude—we began. I asked them to imagine themselves: You are lost at sea on a stormy night. You see a glimmer of light leading you to land. If you row hard, you can make it. Someone waits for you with a warm meal, dry clothes, and a place to rest. I then stated that we would be sharing what we had seen, and proceeded to go around the circle. Adriana shared that she saw Andrew, and that he reminded her of “Patience, stability....old spice aftershave and that sandalwood orange soap he always used..." Unfortunately, Nolan appeared to have a flashback to the time of his injury. While we woke him, Tally continued to agitate other members of the group. He left of his own accord, cursing the session and storming off. Dr. Finn McAlister was present to help with his dismissal. Jamie Gibbs also decided to leave, declaring the session worthless, and stating that I did not care about her. Tally revealed that she saw herself on the shore, and that she didn’t think that she would make it. Lauren went on to reveal she saw her husband and children, that "Kenneth was holding them in his lap. Kenny and Natalie were holding their arms out for me. They need their mother." Tally and Lauren had a positive exchange, and after this I dismissed the group.

Additional: Adriana was receptive to the discussion, if a little bit standoffish. She did not enjoy the comments made by Tallulah, but was at least able to articulate what she saw in her visualization. She did not leave the session and appeared to respectfully participate. She mentioned her step father as the source of her strength.

Follow-Up: I believe Adriana could do with more defined instruction and one on one activities. She seems open to trying, even if a bit resistant.

Therapy Notes: As we began the session, it was obvious that some people were not keen on participating. I explained that we would be listening to different songs in effort to elicit responses and measure our emotions. I wanted to make a connection between the types of memories that could be triggered and the emotions that they feel in an effort to help them understand their control. Some were nonplussed, though Luce stated that he would give it a chance. With “Make someone happy” Joey explained she thought of her father. Moira interjected that it “explained a lot” and we discussed why the song made Joey feel this way. Upon playing “Home”, I asked Elijah about his response. He resisted and then had an emotional outburst, commenting on the death of his wife. He then refused to participate and left the room. “Shake it out” was played next, and I chose Adriana to answer. She had a positive memory of the song, thinking of when she was first promoted. “Last Night I heard Everything in Slow Motion” was the song for Luce, and he explained that it reminded him of “Well, there was this lovely young lady I knew in Tampa who liked music like that. One of the more pleasant times in my life. A real woman, that one - never hit me for trying to be decent.” This elicited a negative response from Eva, as the two had come to blows outside of the session. “Glory Days” was for Eva, and she stated she did not “dwell on the past” and that she didn’t have any such glory days. Moira was last, but refused to participate stating “Because I think this is pretty fucking stupid.” I dismissed the rest of the group and spoke with her one-on-one.

Additional: N/a

Follow-Up: Adriana was overall a good participant in the session. Though she did not appear to enjoy the song selection, she did at least offer up responses when asked. I feel as though she is learning to control her temper and how to express herself.

Session Plan: Session to focus on the topic of self image in correlation to self confidence. Patients will be discussing their own self image, what has caused this perspective, how they feel it has impacted their self confidence, and then patients will discuss as a group ways in which they can improve their self image and thus their confidence, addressing each case individually and sharing ideas.

Session Observations: Miss Clarkson appeared gloomy upon entering the room, though she did nod a greeting to staff once she had taken a seat. Initially she presented herself as very guarded and sarcastic, but after staff intercepted a potential moment for sarcasm, Miss Clarkson was an active contribute in the group session.

Therapy Notes: Miss Clarkson answered inappropriately to introductions, citing her name and describing Hallowsgate, in one word, as 'bullshit.' She was asked by staff not to use vulgarities in the group session. When she was asked to describe how she sees herself as a person, staff intercepted any potential sarcasm by stating that 'with eyes' would not be an acceptable answer. This seemed to derail Miss Clarkson's deflection, and she answered in metaphor, describing herself as a car, lost on a country road, and stuck in mud at the curb. Not scratched or broken down, but stuck and making a mess, splattering mud. Staff asked Miss Clarkson if it was a fair assessment to summarise that she feels lost, stuck and without direction, and asked her to elaborate on how she thinks of herself as a person to whom other must relate. Miss Clarkson answered that she did not know, because she has always found it easier to 'look in than to look out'. Miss Clarkson did show a level of irritation at staff's summary of her metaphor, though it was largely stating her own description back to her minus the imagery. Later in the session, staff opened up the floor to group discussion on how patients could improve or work on their self image. Miss Baker offered a suggestion that Miss Clarkson could engage herself in an activity geared towards helping others, in order to be able to 'look out' more.

Additional: Miss Clarkson must be forced to move past her verbal aggression and sarcasm in order to make progress. She is holding clinicians and peers at an arms length and functioning on a level of denial that is not able to be addressed all the while that she is defending it. Therapy might focus on breaking down these walls of aggression. Miss Clarkson will be assigned to helping to care for the animals, in order to give her an outside focus in helping something else. In the event of a buddy system assignment, Miss Clarkson should be top of the list for female patients.
