Medical History

Do you have any ongoing medical issues for which you require treatment or medication?:

None. Lauren had Chlamydia when she was admitted to our facility, but this has been treated with antibiotics.

Are you aware of any allergies? If so, please list allergy, age of onset and any medications or treatments you require or receive:

Lauren has no history of allergies.

Have you had any surgeries or invasive procedures in the past? If yes, please list reason and approximate age of procedure:

As is our standard procedure, Lauren was given a full health screening when she was admitted to our facility, including urine and blood tests which is how we discovered her STI.

As a child, Lauren had an appendectomy at age ten.

Do you take any medications or supplements daily? Do you follow any treatment plans? Please list medications or treatments, and reasons below:

Lauren takes 10mg of Prozac daily to help with the dysphoria associated with her DPD. She also is required to attend three two-hour sessions of therapy weekly with myself. Despite this, little headway has been made in her case, as our facility is somewhat over-crowded.

Do you use tobacco, consume alcohol, or use any other drugs including street drugs and/or prescription medications not prescribed to you? If yes, please list number of packs a day, number of drinks a day, and/or drugs consumed below:

During captivity, Lauren was kept drugged in order to make her more compliant and docile. By the time she was found, she had been clean for at least three years. Unfortunately, we don’t know exactly what she was on since Lauren doesn’t even know, although we’re pretty sure it was narcotic in nature, possibly an opiate.

Lauren also smoked cigarettes occasionally in the past, beginning shortly before her abduction, although she claims to have quit when she became pregnant.

Psychiatric Screening

Please describe, to the best of your ability, your emotional and mental state of wellbeing:

Lauren has trouble forming and maintaining healthy relationships, and is prone to depressive episodes. She is fixed on being reunited with her ‘husband’, the man to whom she was sold, and prefers being called Mrs. Irving rather than her legal surname. Lately, she has been attracted to strong personalities, and is eager to please those she thinks of as ‘in control’, including some of our more strongly-willed patients. One patient in particular, a Mr. Joseph Heron has a particular hold over her. He has made many attempts to instruct Lauren to do things she knows are against the rules of our facility including but not limited to: trying to give him her own medication, allowing Joseph to abuse her physically, and attempting to engage in sexual conduct with him. She shows considerable difficulty in acting on her own with no instructions from others. She is highly sensitive to criticism.

Have you been diagnosed with any psychiatric or psychological ailments? Please list any diagnoses below, and the treatments or medications prescribed to you. Please include name of medications, dosage, and number of doses per day:

It is my opinion that Lauren suffers from Dependant Personality Disorder, with depressive tendencies. Given her defense of her former owner and captor, she also has a diagnosis of Stockholm Syndrome.

Have you ever been hospitalised or referred to regular outpatient care due to these ailments or associated incidents? If so, please note where, and at roughly what age:

Since her rescue eight months ago, Lauren has been hospitalized here, Brook Lane Health Services.

How have these ailments affected you and your life? Are there any major life instances you feel have been directly affected by these ailments, such as suicide attempts, criminal activities, etc?:

Lauren has spent nearly half of her life in captivity. She has had opportunities for escape prior to her rescue eight month previous, but her Dependent Personality Disorder has left her incapable of taking those opportunities.

Furthermore, since her admittance to out facility, Lauren seems attracted to people she describes as 'in control' or 'strong'. In particular, she seems to be forming an attachment to another of our patients, resulting in opportunities for him to abuse her both physically and sexually. When confronted with her behavior, Lauren is remorseful, but maintains that Joseph 'knows what's best', and claims that he wants to help her reunite with her husband.

What is your social life like? Do you have many friends or relationships? How are your family relationships?:

At the time of her rescue, Lauren’s only relationships were with her captor, who she considered her husband, and their two children. Since then, she is polite to others, but mostly resists making any real connections. She has been reunited with her family on a few occasions, but even this has failed to produce any meaningful relationships.

Upon admittance to our facility, Lauren formed a bond with Joseph Heron, another of our patients with an abusive personality. After several infractions of the rules, Lauren and Joseph were no longer allowed to continue their relationship. After severing the relationship, Lauren seemed lost and claimed that she needed someone to tell her what to do. She is uncommonly obedient, although she lacks the initiative to act without instruction.

Do you believe your life circumstances have contributed to any ailments? If so, what circumstances, and why do you feel they have contributed?:

When Lauren was fourteen years of age, she was approached by a man at the mall who claimed to be an agent for a modeling company. He enticed Lauren to come by for a ‘test shoot’. This was a ruse. In fact, the man was working for a human trafficking ring. Lauren was transported four hundred miles from her home to avoid recognition, and she spent the next several years being abused by various ‘clients’. About four years ago, she was purchased by one of her more frequent clients, a doctor, for his personal use. After he got her off the drugs she’d been given by her former owners to keep her more docile, he kept her out at his secluded second home, where he delivered the two children he fathered with her. The doctor was suspected on an unrelated charge, and when the police searched the house, they found her and the children huddled together in the basement, hidden inside a large wardrobe that seemed to have been placed there for that purpose.

If you could change one past event that has happened to you, what would it be, and why?:

When asked about her greatest regret, Lauren answers, ‘I wish I could have been more quiet.’ This is in reference to her rescue.

Do you wish to rehabilitate from your ailment(s)? If so, how do you feel this would best be accomplished?:

Lauren denies having any ailments.

Environmental History

Where did you grow up? Please list the location(s) and describe what it was like growing up there:

Lauren grew up in Baltimore, Maryland. Her parents were lower-middle class. Her childhood before her abduction seems to be fairly standard.

At age fourteen, Lauren was abducted and taken to live in a renovated industrial space near Boston.

At nineteen, she was purchased by Dr. Kenneth Irving and taken to live in his second home in Connecticut. While there, she gave birth to two children, Kenneth Jr. and Natalie. Her ‘husband’ continued to visit prostitutes, and she also contracted asymptomatic Chlamydia. Eight months ago, police found her and the children hidden in the basement while they were searching the house because of an unrelated case in which Dr. Irving was implicated.

What was your family life like? Did you spend much time with your parents? Do you have any siblings? If so, what are your relationships like?:

Lauren describes her family relationships as ‘neutral’, and has seen them only a few times since her rescue. She rarely speaks of her life before her abduction. Interviews with her family reveal that they were rather close-knit. She has three siblings; two sisters and a brother. Lauren was second eldest in her family.

What was school like? Did you have any problems? Did you enjoy school? What were your grades like?:

Before her abduction, Lauren was an average student, maintaining a B average in most of her classes. Since her rescue, she has been indifferent to any suggestion of earning her GED, although when left to her own devices, she does seem to enjoy reading.

Did you engage in any extracurricular activities, such as academic, artistic, or sporting clubs?:

Lauren is a talented seamstress, having made most of her own clothing as well as her children’s while she was held captive. Prior to her abduction, Lauren seemed to have had artistic aspirations.

What was work like? Did you have any problems? Did you enjoy your work? What were your coworker relationships like?:

The only ‘work’ Lauren has ever done has been forced prostitution. Occasionally she was used to make pornographic materials. She refers to this as her ‘modeling career’. She talks of a few friends she had there, mostly other prostitutes with whom she shared sleeping quarters.

Have you ever been convicted of a crime or misdemeanor? If yes, please explain, list conviction, and list any sentences associated with convictions:

Lauren has no criminal record. Her involvement with law enforcement has been as a victim.

Lastly, please tell us about yourself. How do you feel about yourself and what you have done with your life? If you have committed crimes, how do you feel about those now? What are your hopes for the future?:

I had a happy life. I was married to a good man and I had two beautiful children. I want that life back. I never did anything bad, did I?

Lauren’s children have been placed in her elder sister’s care for the time being. Lauren knows this, and harbors some resentment toward her sister, perceiving that she is trying to steal her children. Lauren doesn’t love her husband exactly, but she knows how bad it could have been, and she is supremely grateful to him for how well he treated her, despite the fact that she spent the first year of their ‘marriage’ locked in the basement.

Therapy Notes: In today's session, the topic was positive and negative emotions, and the patient's experience with each. I had them write down both the most positive and negative emotion they experience and mix them in with a few emotions I added myself. They were then asked to draw out a slip, and describe their experience with whatever was written on the slip. Rather than have them draw one positive and one negative, I left it to chance what they would get, as I am interested in hearing about all emotions they feel and how they affect them.

Lauren was quiet when she entered, and was soft spoken throughout the session. She answered when asked questions, only speaking out of turn once from what I heard. When asked to give her name and where she was originally from, she stated that she was from Connecticut, but grew up on Boston. I find it interesting that she generalized with one but not the other, especially since she lived with her captor in Connecticut. Lauren's first word was 'Obsession' and I was not expecting the answer she gave. She spoke of her teenage years, presumably before she was kidnapped, and how she'd been obsessed with boy band and celebrities. She described having posters of them in her room, and also stated she wanted to be famous. We will be discussing the nature of obsession in individual therapies, and how it might relate to her relationship with her captor.

Lauren's second word was 'Fear' and she did relate this emotion to the time she spent hostage. She said that "Fear is sitting in the basement with two toddlers praying that no one can hear you" and went on to add that she also feared not knowing when she will see her children or husband again. She did not mention being afraid when she was taken, however, and this is another point that I would like to cover with her in therapy.

Additional: I mentioned early on the notes that Lauren spoke out of turn, although I would hardly call her disruptive. She responded to Joey's word "Empty" by saying that she was empty too. I suspect it ties in with her Stockholm Syndrome, and that she feels empty without her captor and children.

Follow-Up: I will be meeting with Lauren soon to discuss the words she chose and her answers to them. No medication change required at this time, and I recommend that in future sessions she be encouraged to speak more.

Therapy Notes: I began the session welcoming each participant and allowing them to choose a seat in a circle within the recreation room. Tally was disrespectful from the start, referring to me by my first name “Rosie” although was altogether pleasant otherwise. Adriana appeared disinterested in the session, as well as Nolan. Lauren was quiet and unassuming upon entering. Jamie appeared irritated and disinterested. We began the session introducing ourselves, and it seemed most were tired of this exercise (as it appears in most sessions—there should be a note regarding who has worked with whom). Shortly thereafter, we began the visualization exercise. After explaining to several members of the group what visualization was—with some help from Tally, albeit critical and rude—we began. I asked them to imagine themselves: You are lost at sea on a stormy night. You see a glimmer of light leading you to land. If you row hard, you can make it. Someone waits for you with a warm meal, dry clothes, and a place to rest. I then stated that we would be sharing what we had seen, and proceeded to go around the circle. Adriana shared that she saw Andrew, and that he reminded her of “Patience, stability....old spice aftershave and that sandalwood orange soap he always used..." Unfortunately, Nolan appeared to have a flashback to the time of his injury. While we woke him, Tally continued to agitate other members of the group. He left of his own accord, cursing the session and storming off. Dr. Finn McAlister was present to help with his dismissal. Jamie Gibbs also decided to leave, declaring the session worthless, and stating that I did not care about her. Tally revealed that she saw herself on the shore, and that she didn’t think that she would make it. Lauren went on to reveal she saw her husband and children, that "Kenneth was holding them in his lap. Kenny and Natalie were holding their arms out for me. They need their mother." Tally and Lauren had a positive exchange, and after this I dismissed the group.

Additional: Lauren was particulary quiet but respectful during the session. There were a few times when she spoke out against the fights that erupted between Tally and others, but for the most part kept to herself.

Follow-Up: Lauren still relates her source of strength as her husband. She needs to develop a healthier sense of her own identity. I believe that she would do well with self-confidence building activities, as well as exploring
