Medical History
Do you have any ongoing medical issues for which you require treatment or medication?:Aside from his psychological afflictions, Mr. McAuliffe currently suffers from no medical issues that require treatment.
Are you aware of any allergies? If so, please list allergy, age of onset and any medications or treatments you require or recieve:Mr. McAuliffe is mildly allergic to animal dander, and takes over-the-counter allergy medication when going anywhere that might require him to be around pets. He has a particularly strong reaction to horses.
Have you had any surgeries or invasive procedures in the past? If yes, please list reason and approximate age of procedure:Four years ago, Mr. McAuliffe was involved in a car accident that required moderate reconstructive surgery to his right clavicle, scapula, and humerus. Incidentally, it was shortly after this that his relatives and friends claim his paranoia and delusional behavior began in earnest.
Do you take any medications or supplements daily? Do you follow any treatment plans? Please list medications or treatments, and reasons below:Mr. McAuliffe is allowed mild narcotic painkillers upon request to alleviate pain caused by the injuries mentioned above, but otherwise takes no non-psychiatric medications on a regular basis.
Do you use tobacco, consume alcohol, or use any other drugs including street drugs and/or prescription medications not prescribed to you? If yes, please list number of packs a day, number of drinks a day, and/or drugs consumed below:Mr. McAuliffe remains a regular smoker, although since being committed to our institution he has reduced his intake from one pack a day to about half that. I had originally thought this to be a positive sign, but he simply expressed to me a fear of being abruptly cut off, and a desire to lower his dependence as a preemptive strike, rather than any intention to improve his quality of life.

Psychiatric Screening
Please describe, to the best of your ability, your emotional and mental state of wellbeing:In recent months Mr. McAuliffe has become unable to function properly due to extreme paranoia, trigger-specific fear (which he most often copes with through violence), and what I believe to be an underlying sense of guilt and personal inadequacy.
From what I have gathered, his childhood experience was authoritarian in nature, bordering on and lapsing into outright abuse. His mother seems to have been the dictator, so to speak - the one who laid out strict rules to be followed - while his father functioned as an enforcer, doling out disproportionate punishments for misbehavior and mistakes. The abuse, however, seems to have stemmed from a misguided attempt at pushing he and his twin brother to be successful, rather than outright maliciousness. He obviously shared a very close bond with his twin, Marcus, of whom he still occasionally speaks glowingly despite the fact that his persecutory delusions center on him. It is my belief that they provided a respite for each other as children, and were each other's only source of affection; receiving none from their parents, they overbonded to one another to the point of codependency.
When Mr. McAuliffe and his brother were seven years old, they were involved in a car accident that claimed Marcus' life, but left Henry largely unharmed. Mr. McAuliffe was trapped inside the vehicle for approximately thirty-six hours before the site of the crash was discovered, and the experience has obviously left a very strong impression on his psyche. After his recovery it seems that he felt his parents blamed him for his brother's death; whether this is true or not, he seems to believe that he was the inferior twin, and that the world was somehow cheated in that he survived, and Marcus did not. A perfectly understandable case of survivor's guilt, if not for the fact that he came to believe the ghost of his brother blamed him for the incident as well. When asked, a childhood friend stated that Mr. McAuliffe confided in her several years after the fact that he believed Marcus was angry with him for not doing anything to save him while trapped within the car, and that Marcus had been following him since, giving him nightmares and following him to school.
Mr. McAuliffe's delusions apparently did not begin to noticeably impair him until the car accident in which he injured his shoulder. According to his own statement, he swerved to avoid a pedestrian who appeared out of nowhere (whom he later claimed to have been his dead twin), and was pinned by debris in a manner almost identical to that of his brother decades earlier. He came to the conclusion that after years of passive observation, Marcus had attempted to murder him, and he afterward began to withdraw from his career and relationships, seeming increasingly paranoid as time went on. Friends have described him as constantly looking over his shoulder, abruptly ceasing conversations for no apparent reason, and making excuses to flee from public places with unknown motivations. He was committed to our care six months ago when, after having refused calls from friends and failed to pay his bills for weeks, he was found in a motel in a severe state of personal neglect. The police were called to arrest him when he physically assaulted a maid for uncovering the mirror in his room while cleaning it, and it quickly became clear that he was mentally unstable.
Mr. McAuliffe, as with many paranoiacs submitted for treatment unwillingly, has difficulty developing trust toward medical professionals, and has not responded well to therapy. He also expresses dislike for being medicated, but usually does not resist taking them.
Have you been diagnosed with any psychiatric or psychological ailments? Please list any diagnoses below, and the treatments or medications prescribed to you. Please include name of medications, dosage, and number of doses per day:At present, I can most conclusively diagnose Mr. McAuliffe with Delusional Disorder, persecutory subtype. As his paranoia is specific rather than general, I have not added PPD to his diagnosis, though I welcome a second opinion. It should also be noted that Mr. McAuliffe suffers from severe Spectrophobia; as the source of his paranoia is his twin, he is extremely opposed to seeing his own reflection, and will go out of his way to avoid reflective surfaces. He is not generally unreasonable if not pressed, and will likely request mirrors in his vicinity be removed or cover them as a first option, but if forced into their presence he will not hesitate to break them, even if this will result in injury. In addition, though he does not suffer from insomnia, he often attempts to force himself to stay awake to avoid nightmares, where he believes Marcus can most easily harm him; he has taken to biting himself to stay alert, a habit that has been difficult to discourage.
I began treating him with Risperidone, but this exacerbated his sleep deprivation and caused tremors and muscle aches. He is currently prescribed 6mg Pimozide once daily, taken in the morning. I have also taken to giving him 2mg of Eszopiclone at night when his reluctance to sleep affects him too strongly, but I am reluctant to risk him developing a sedative dependance, as he has stated to me that the nights he takes it are the only ones in which he doesn't dream.
Have you ever been hospitalised or referred to regular outpatient care due to these ailments or associated incidents? If so, please note where, and at roughly what age:Mr. McAuliffe was committed to McLean Hospital six months ago, as stated above.
How have these ailments affected you and your life? Are there any major life instances you feel have been directly affected by these ailments, such as suicide attempts, criminal activities, etc?:As previously stated, Mr. McAuliffe has become unable to function normally as a result of his delusions. He is constantly anxious, prone to becoming aggressive when anxiety lapses into fear or his delusions are questioned, and unwilling to bond socially for fear of Marcus' alleged persecution extending to others as well. I do not believe he is currently capable of living on his own without causing harm to himself or others.
What is your social life like? Do you have many friends or relationships? How are your family relationships?:Mr. McAuliffe's relationship with his parents is distant and resentful at best, but up until recently he apparently had an active social life. He has deliberately distanced himself from friends out of a perceived need to protect them, and resists forming new relationships, which has made therapy difficult.
Do you believe your life circumstances have contributed to any ailments? If so, what circumstances, and why do you feel they have contributed?:I believe Mr. McAuliffe's childhood circumstances and trauma to have actively contributed to his illness; he will need to address and come to terms with them before he can truly begin to recover.
If you could change one past event that has happened to you, what would it be, and why?:I asked Mr. McAuliffe this question directly, and he stated that he would have died in Marcus' place, saying it would have been better for all involved.
Do you wish to rehabilitate from your ailment(s)? If so, how do you feel this would best be accomplished?:Mr. McAuliffe has yet to acknowledge that he suffers from a psychiatric disorder, but he has stated before that he does wish to be rid of Marcus, and that he is tired of being afraid. If he can confront the fact that he is delusional, he might well be receptive to treatment.

Environmental History
Where did you grow up? Please list the location(s) and describe what it was like growing up there:Mr. McAuliffe was raised in Boston, Massachusetts, in an upper middle-class environment that encouraged success to the point of being intolerant to anything less than perfection. He was held to a militaristic standard, and had few friends until he was an adult from what I understand.
What was your family life like? Did you spend much time with your parents? Do you have any siblings? If so, what are your relationships like?:He received little parental affection, most if not all of his positive human interaction coming from his twin brother, with whom he bonded through shared emotional and physical abuse. Even today he seems more inclined to trust other men than women, especially those with assertive personalities, to whom he may turn emotionally hostile if pressured.
What was school like? Did you have any problems? Did you enjoy school? What were your grades like?:Mr. McAuliffe describes Marcus as the intellectual, and himself as the athlete; his grades were decent, but never stellar, and he suffered for it at home, which I believe contributed to his belief that he was the lesser compared to his brother. He says that he was fairly popular, but never close to anyone.
Did you engage in any extracurricular activities, such as academic, artistic, or sporting clubs?:Mr. McAuliffe was involved in both baseball and track throughout middle school and highschool, and running is still a hobby he professes to enjoy.
What was work like? Did you have any problems? Did you enjoy your work? What were your coworker relationships like?:Mr. McAuliffe worked as a physical therapist for over ten years after graduating from college, with a focus on rehabilitating those that were crippled in accidents - his motivation, I think, being clear. He was described by coworkers as dedicated and empathetic up until he own accident, after which his performance and attendance began to decline, until he was eventually fired a year before he was committed to McLean. By all opinions his departure was regretted by all involved.
Have you ever been convicted of a crime or misdemeanour? If yes, please explain, list conviction, and list any sentences associated with convictions:The motel maid whom Mr. McAuliffe assaulted agreed not to press charges upon learning he was being committed to a psychiatric hospital for treatment. Otherwise, his criminal record is clean but for a few speeding tickets.
Lastly, please tell us about yourself. How do you feel about yourself and what you have done with your life? If you have committed crimes, how do you feel about those now? What are your hopes for the future?:During my time with him I have been given the impression that Mr. McAuliffe has a very low sense of self-worth, which he conceals with humor or aggression depending on the situation. Up until his accident he was apparently at peace with his profession and social life, and he obviously regrets what has become of his life since then, though his denial prevents him from doing anything to change it. I believe that with proper treatment he could be happy and successful again, but that may be a long time coming.

This is where you can tell us all the things your character wouldn't put on an application, or others don't know.:Most of who and what Henry is has been stated up above, but he is the only one that knows that Marcus didn't die on impact during the car crash. He bled out over several hours, and Henry had to watch and talk him through it, which scarred him deeply. He has also been considering suicide as an alternative to the constant fear he has been living with for four years, thinking it might appease his brother, but has been unable to steel himself to try.
I should also note that his brother isn't
actually haunting him - he has no special sixth sense or what have you unless purchased in-game. Just as a disclaimer.
