Initially, Miss Janine Zeeger did try to fill out this application herself, she found it extremely frustrating and the papers did not survive the experience. Her inability to keep her reality straight is something that Janine struggles with and deeply resents. – Dr. Sansen _____________________________________ Medical History Do you have any ongoing medical issues for which you require treatment or medication?:Miss Zeeger continually tests low for vitamin D. She has since she was a teenager. Are you aware of any allergies? If so, please list allergy, age of onset and any medications or treatments you require or recieve:Janine is allergic to any kind of peppers, since she was very young. She says her mother thinks she was 3 when she first had a reaction: breaking out in hives. It's grown less severe over the years. Have you had any surgeries or invasive procedures in the past? If yes, please list reason and approximate age of procedure:Her wisdom teeth were removed at age 17. Do you take any medications or supplements daily? Do you follow any treatment plans? Please list medications or treatments, and reasons below:A women's multivitamin, the vitamin D supplement mentioned above and a low dosage birth control pill. Do you use tobacco, consume alcohol, or use any other drugs including street drugs and/or prescription medications not prescribed to you? If yes, please list number of packs a day, number of drinks a day, and/or drugs consumed below:Given that Janine's been in and out of institutions for years she hasn't had much opportunity for experimentation. She says doesn't recall ever using street drugs or prescriptions not prescribed to her. Though shortly after stating that she began to second guess herself. She said she has drank in the past. _____________________________________ Psychiatric Screening Please describe, to the best of your ability, your emotional and mental state of wellbeing:There is rarely a day that goes by in which Miss Zeeger manages to maintain a happy mood. She is most frequently depressed and angered by her current position and developing mental state. She is often confused/disoriented and her struggle to maintain an understanding of the world around her is a constant fight. She finds it hard interacting with her immediate surroundings: constantly feeling numb and separate from the things around her. Janine often feels that her dreams run together with her real life. Have you been diagnosed with any psychiatric or psychological ailments? Please list any diagnoses below, and the treatments or medications prescribed to you. Please include name of medications, dosage, and number of doses per day:At fifteen Janine was first diagnosed with Somnambulism and REM Behaviour Disorder, however according to her and her parents she was exhibiting mild symptoms of RBD also side her sleepwalking since she was a child. At the age of nineteen she began trying medications for RBD as her episodes became more serious and aggressive. Over the years she's tried many combinations but is currently taking Clonazepam (1mg daily at dinner). This does lessen her RBD episode frequency but she continues to sleepwalk. As the years have gone on her RBD has begun to cause serious problems in her waking life. The condition began to develop about seven or eight years ago (she had been bottling up certain concerns and problems for sometime and withholding information), but six years ago she was given a solid diagnosis of Depersonalization Disorder. While she's awake she has difficulty connection with herself and the people around her. She also has a history of confusing her dreams with reality. At this juncture we have experimented with a few medications for her DP (Sarafem, Naloxone, Revia) but have yet to hit on a successful combination with her Clonazepam. Have you ever been hospitalised or referred to regular outpatient care due to these ailments or associated incidents? If so, please note where, and at roughly what age:Janine began outpatient treatment at fifteen at a sleep lab, Rocky Mountain Heart Sleep Centre. She worked with them off and on for a few years. Shortly after her high school graduation she checked in as an in-patient. She had a firm desire to kick her sleep disorders, feeling that they were running her life. At 22, she transferred to our facility–Wings of Change Mental Health Centre–seeking a more aggressive treatment. How have these ailments affected you and your life? Are there any major life instances you feel have been directly affected by these ailments, such as suicide attempts, criminal activities, etc?:Janine has harmed in the past out of confusion and frustration, thought we don't believe it to be intentional so much as accidental. She has always sought out health for herself, feeling that she is incapable of having a functional life with her disorders. What is your social life like? Do you have many friends or relationships? How are your family relationships?:During her schooling, Janine had a few close friends, but has lost touch with them over the years. While in inpatient care Janine has made a few friendships with other patients, even a pair of brief romances. Her closest friend is a younger woman named Lucy at our facility who is being treated for BDD. She is close with her family and relies on them heavily. Presently, Janine is having difficulty maintaining her relationships in a healthy manner, her confusion between dreams and reality have caused her to attribute dream events to the real human counterparts causing animosity. We also had to reassign her from her former therapist to myself due to these confusions and the distrust they cause. Also, her detached feelings with herself and her body have caused issues connecting with other people. Do you believe your life circumstances have contributed to any ailments? If so, what circumstances, and why do you feel they have contributed?:Her sleep disorders have very clearly contributed to her development of RBD. Also her RBD has aggravated the sleep disorders in turn, due to confusion of time and reality. She often refuses medication, thinking she's already taken it. If you could change one past event that has happened to you, what would it be, and why?:The scarring on Janine's shoulder is the result of an attempt to exit her parents' home through a closed sliding glass door when she was home for Christmas at 20 during what was thought to be a time of improvement. She could have easily been seriously injured in the episode and Janine frequently describes how scared she was walking up in pain and surrounded by glass, her father also cut up and trying desperately to calm her down. More recently much of her regret is focused on events that are not one hundred percent accurate to reality. She mentions hurting Dr. Morn, unsure of how exactly, but feels a deep grief. Dr. Morn stated that nothing out of the ordinary happened in their sessions recently and we have been unable to alleviate her emotions on the subject. Do you wish to rehabilitate from your ailment(s)? If so, how do you feel this would best be accomplished?:Miss Zeeger desperately wants to be healthy and happy, she frequently expresses a desire to lead a normal, connected life. With our current difficulties treating her and the fact that our facility is closing once we've relocated our patients that need continued care we are looking for a facility that can provide a more intensive treatment for Janine. _____________________________________ Environmental History This portion was complied with interviews with Janine at an earlier date as well as interviews with her family. Where did you grow up? Please list the location(s) and describe what it was like growing up there:Janine grew up in Missoula, Montana, A mountain town. Her childhood was fairly healthy and quite happy. She was raised in a comfortable middle-class family and was given proper care and attention. What was your family life like? Did you spend much time with your parents? Do you have any siblings? If so, what are your relationships like?:Janine is the youngest of three. She has two elder sisters: Melissa and Tawny. Melissa (the eldest) and Janine have a somewhat strained relationship while their mother admitted likely has to do with Melissa resenting the extra attention Janine received growing up. Tawny and Janine however have a very strong relationship and keep in regular contact. Tawny's bedroom was across the hall and she often cared for her sister or alerted their parents during Janine's episodes. Miss Zeeger's parents have an excellent relationship with her, and they support her fully in treatment. Even if it means she leaves the state. What was school like? Did you have any problems? Did you enjoy school? What were your grades like?:Janine did well in her early schooling years, in high school however her sleep disorders began to effect her waking life. Her grades slipped, but she did manage to graduate with her class. It was an important goal to her as she grew up with the same group. Did you engage in any extracurricular activities, such as academic, artistic, or sporting clubs?:Janine participated in her schools vocal group until she was 17 and found it difficult to devote time to it. She also writes a fair bit, I understand when she was younger she did a lot of journaling and blogging. What was work like? Did you have any problems? Did you enjoy your work? What were your coworker relationships like?:Janine has had a few short lived retail jobs: during high school and during a few small bouts of outpatient care. When she was young she also used to do some work for her parents as they own a bed and breakfast next door to their home. She and a few of our patients here used to do group trips to do charity social calls to a local seniors home, but this was before her DPD developed. Have you ever been convicted of a crime or misdemeanour? If yes, please explain, list conviction, and list any sentences associated with convictions:No, Miss Zeeger has never been charged with anything. Lastly, please tell us about yourself. How do you feel about yourself and what you have done with your life? If you have committed crimes, how do you feel about those now? What are your hopes for the future?:Earlier on in her treatment, Janine often talked about regaining her life and going back to school. As her condition worsened she seems to have downgraded her ambitions. Now she simply expresses a desire to connect and feel at ease. _____________________________________ Out of Character Section What are they not telling us? What secrets do they have to hide? What back story are we not hearing? This is where you can tell us all the things your character wouldn't put on an application, or others don't know.:Janine is a victim of Gaslighting (form of psychological abuse in which false information is presented with the intent of making a victim doubt his or her own memory and perception. - Wikipedia). Her confusion in regards to her past therapist Dr. Phillip Morn is intentional. As he began to notice her rising confusion he began monitoring her closely. Eventually he began experimenting with her medications. One day, a combination had her especially out of it and he took sexual advantage of her state. Since then it's been a long string of abuse and manipulation. Anytime she questioned him he'd do everything in his power to make her feel guilty for it: telling her she wasn't following through on things she agreed to like taking her medication or showing up to sessions. Anytime she's said anything about sex or the possibility that something of that nature happened between them, he's convinced her that she put him in a bad position or tempted him, that it should be kept quiet. In his desperation to cover his crimes and keep her off balance he'd even toy with her mind in unrelated ways. Claiming not to have heard a sound that they both did, staying she imagined it. | THERAPY NOTES _____________________________________ Date:December 2, 2012 Attending: Dr. Finn McAlister Session With: Dr. Finn McAlister Session: Group Therapy
Therapy Notes: Today’s session was low key and involved helping Hallowsgate look more festive for the holidays. This involved setting up several Christmas trees in the dining room, decorating them and ending in Peppermint hot chocolate and gingerbread men. Each of the patients entered calmly, and I asked for each to introduce themselves starting with Moira who did not comply, however everyone else did state their name. I split the group into twos, with Janine Zeeger and Henry McAuliffe into putting up one tree near the door, Mallory Bates and Harlow Jacobs putting up the tree near the kitchen, and Moira Hanley and Myself separating garland to decorate the dining hall. Moira Hanley refused to get up from her seat to assist, and Dr. Benjamin Graham, Moira’s attending stepped in to assist. I then went about sorting ornaments for the group, as well as lights, divvying them up between the two groups setting up the tree. Janine and Henry worked well together, and Henry seemed in somewhat decent spirits. He stated that this activity was better than sitting in his room, or talking in a circle with a bunch of crazies. Janine also seemed in high spirits, especially when she realized we would be decorating for the holidays. During this discussion with Henry and Janine, an outburst occurred between Harlow Jacobs and Mallory Bates. As I did not see the altercation directly, I cannot place blame, however, Mallory did shove the lights at Harlow and into his chest. From my understanding, Harlow believed Mallory was attempting to take the lights from him, and expressed his frustration in a loud outburst. Harlow exclaimed that Mallory tried to take the lights, and then stated that Mallory threw the lights at him, and called him a name. However, because I did not witness the altercation face to face, I cannot back up or deny this. Due to Harlow's delivery and outburst, however, he did face punishment after a warning of calming down and working as a team, in which he refused to pick up the lights and help. Harlow also stated that he didn't like Christmas, and he didn't want to put together the stupid tree. He also stated that he told Mallory to leave him alone. Due to this, and his inability to calmly answer why he hollered and what happened,he was placed on dish duty for the afternoon in the kitchen for this outburst. Mallory refused to tell his side of the story, however, he was not sent out. This will be discussed in a later individual therapy session, and I will check back up with Harlow again once he has calmed down. Towards the end of the session, I allowed the patients to enjoy gingerbread men and hot chocolate. Moira worked through the session with Dr. Graham and earned her hot chocolate and cookie, however, she did not take the cookie, and simply took a hot chocolate back to the table for Dr. Graham. Additional: Janine and Henry worked well together as a pair. Follow-Up: She seemed to enjoy the tasks and when given positive feedback, she did not react positively or negatively, though due to her condition, I believe positive feedback to be beneficial, rather than no feedback.

| INCIDENT REPORT _____________________________________ Date: 11/28/12 Attending: Dr. Finn McAlister
Notes: On the evening of November 28th, I was on my rounds of the women's wards when Janine came running down the hall, clearly in distress. Unable to tell whether she was awake or asleep, I reached out for her as she passed by, but she ran right by me. I pursued her and was able to stop her before she got much further down the hall. Not recognizing me, she lashed out and scratched by arm in an attempt to get away, but no lasting damage was done and I managed to calm her shortly thereafter. At that point I realized she'd been sleepwalking, clearly running from something in her dreams. When she woke up she was apologetic for her behavior and visibly shaken. Action Taken: Once reassured that Janine was awake and subdued, I let her accompany me for awhile on my rounds. I suggested escorting her back to her dorm, but she didn't want to return right away, distressed by what had happened and claiming she'd only disturb her roommate. I agreed with this and decided that it was unlikely she'd be able to go back to sleep right away. Janine was pleasant and soft-spoken and stayed with me the whole time, seeming grateful for the distraction of a walk. Follow-Up: Janine didn't want to talk about what had happened in her dreams, and I didn't push her. I think it would be beneficial, however, to touch upon this incident in her next individual therapy session, and see what might be done to help her sleep better in the future.
