Please explain why you are applying for the position(s):
To say it most simply, my Canadian work visa is about to run out, so I'm looking for opportunities in the U.S. Believe it or not, but your facility would be my attempt to work someplace less isolated. I enjoy a small, close-knit work environment but I'm looking for a change of pace. I've been working up north for five years now and want to spread my wings so to speak.

Why do you feel you would be the best candidate for the position(s)?:
As I said, I enjoy and work very well with a small group. Seeing that this is a similar set-up I think you'll find I'm very effective. Also, up north, especially in disenfranchised areas like Fort Simpson, depression is a very common illness. I've probably seen more of my share of suicide attempts and cases. So I'm well versed in dealing with the symptoms and the aftermath if need be.

Do you posses the necessary qualifications for the position(s) for which you are applying? If yes, please list them below:
I have my BS from Eastern Washington State University, my MD from the University of British Columbia and worked as the lead physician and eventually head of medicine for Fort Simpson Hospital.

What other qualifications or skills do you possess which you feel will aid your performance in the chosen position(s)?:
I'm well versed in working with certain mental illnesses as I mentioned above. As well as working with people in less than ideal living situations and isolated conditions.

Have you worked in this type of position or facility before? If so, where and when?:
My position at Fort Simpson Hospital was very similar, leading a small, tight-knit team. We were never a big enough center to have a full-fledged mental health sector but that's a next experience that I look forward to engaging.

Please describe your most recent employment experiences, including your positions and responsibilities there:
Fort Simpson Hospital is a remote northern clinic in Canadian's Northwest Territories. Due to the harsh conditions and emotional toll of life up north we experience a lot of turn around staff-wise. While unfortunate, this allowed me to rise up the ranks very quickly until I was chief of medicine for the past two years. I lead a team of two other doctors and six nurses. We had no specialists and only one specific surgery staff. Any patient needing more specific care was flown out to Yellowknife's Stanton Regional Hospital.

Before I came to Fort Simpson I completed my residency at Creston Valley Hospital in Creston, British Columbia in 2004.

How do you think your previous education and employment experiences will aid your performance within the chosen position(s)?:
Despite having only a marginal experience with mental health patients I think the rest of my experience is tailor-made for a facility like your's. I'm a great leader for small teams and take a very personal interest in every case I can manage.

What methods would you employ in order to relax during times of stress, and how effective do you think these have been in the past?:
I stretch, some might call it Yoga I guess. Really I just like to take an hour stretch everything out, give myself a neck rub or something. Just decompress and not think for a while. It's always been fairly effective for me.

Negative: I have tunnel-vision in a way. I tend to focus on one aspect of my life at a time. I'm Arthritic in my legs, the lasting damage of an accident in my youth. I have a Black Thumb.

Positive: Focused, Friendly, Inquisitive

Describe one event in your life that you would change if you could, and why:
Recently, one of the nurses working under me at Fort Simpson Hospital committed suicide. I deeply regret I didn't see it coming as I would have done just about anything to keep her here. She was a wonderful woman and an even better nurse. She simply had the unfortunate level of empathy that makes you take on just about everyone's pain.

If a superior gave you a task to complete, to which you were wholly, morally opposed, what would you do?:
Discuss it with them. Communication is always key. I'm sure a mutually agreeable solution could be reached.

If you caught a friend and co-worker stealing from your place of employment, breaking rules, and/or endangering the safety of others, what would you do?:
Talk to them if the behaviour could potentially be rectified. If not, I'd have to talk to a superior about it.

If your application for employment to Hallowsgate Hospital is approved, will you agree to sign a full non-disclosure agreement, knowing that you would be unable to talk about any treatments or the patients with anyone not also employed by Hallowsgate Hospital?:

Environmental Screening

Where did you grow up, and what was it like there?:
Although I was born in Cebu City, Philippines, I grew up in Spokane Washington. My parents immigrated when I was three. It's a lovely city. Very friendly, very clean.

What are/were your social interactions like? Do/did you have many friends and/or relationships?:
I tend to be closest with my family and my co-workers. I don't tend to have many friends outside my place of work, but that probably has more to do with the relative isolation of places I've worked in the past. My social interactions are mostly amicable. I don't tend to make any enemies.

What prompted you to pursue the career path you are now following?:
When I was in high school I was a gymnast. In fact, I was looking to qualify for the Olympics in a year or two. It was my life. In my senior year I was stopping into a local mall on a dark winter evening to pick something up and jogging across the busy street to my destination. I wasn't paying close enough attention and ran out in front of a car. It was a bad accident and I spent the next few months in hospital. I'd broken my right femur and kneecap, as well as a couple of ribs and suffered some internal bleeding. All in all making it quite impossible for me to continue my Olympic dreams.

It was during that time that my focus shifted to something else. Instead of focusing on my body and what it could or could not do, I started focusing on body's in general and why they do what they do. I became fascinated by the idea of becoming a doctor during my stay at the hospital and I guess the rest is history.

Out of Character Section

What are they not telling us? What secrets do they have to hide? What back story are we not hearing? This is where you can tell us all the things your character wouldn't put on an application, or others don't know.:
To call Dr. Lim a workaholic is too gentle. She tends to have little to no social life. She dedicates all her time to her place of work and the tiny remainder to unwinding alone at home. She's friendly and likeable, but she's simply not one to take a step out of her comfort zone to ask someone to hang-out after hours.

This has resulted in her not having sex with a partner in the last 11 years. And the last instance was a disappointing one-night-stand. Enjoy.

Also, she does not use her given name. Filipino culture is big on very Christian names and honouring with names so Dr. Lim is Josefina Maria Lim the fifth. She goes by Tuesday, her nickname since birth as it was the day on which she was born.
